Your drivers will also find it easier to input state abbreviations as we’ve included a full list on the right side of the form. Right under the driver signatures are boxes for origin and destination as well as the dates. There is also spaces to track the odometer reading throughout each state in addition to logging the miles. Plugging in dates is also easier as there is one box for the month and another for the date. Each section has a bottom line for calculating the total trip miles and expenses. The bottom verbiage reads “State laws require that an accurate record of miles drive and fuel purchased in each state be kept fort his vehicle”.
